Sent a voluntary acknowledgment of verification of employment to me with no explanation. Do not know these people

I do not know who this is. I have had someone repeatedly calling my job with threats that include fraud, warrant for my arrest, falisifying information, etc. I asked them to send me a signed copy of what they were trying to collect and they just sent a voluntary acknowledgment of verification of employment. I did not complete this form nor return it. Someone called my home and talked to a 16 year old visitor and told him that owed money and that he would have me arrested if he didn't put me on the phone. I was not home at the time but apparently he discussed the supposedly debt in detail with this minor.

If i owe something, i think that the company i owe needs to contact me not these foreigners who call and make terrible threats.
