Charge card criticism

I noticed this US credit and ripped my credit record. A Revolving consideration that I never exposed.
No card sent. 500, quantity 12.
Claims I popped in Jul. I had been in clinic at that time.
On my statement it claims customer support
One Millennuim Push
Uniontown PA 15401
Claims US Plantinum Card after I looke up US credit.
Also, noticed there is another US Credit in California
telephone number 727 226 2529.
I've had medical expenses and needed to state bankruptcy and of course indentity theft in past.
I'm tired of those individuals destroying peoples lives.
It-they read this, I would recommend they remove off statement as sealed.
I've talked to my lawyer.
