Sallie Mae Repayment of student loan

Never try to repay a loan with Sallie Mae after it has been in forbearance. They wil NOT send any information telling the amount we owe at this time and they will NOT give you the option of setting up a payment plan. The only communication we can get is a form for us to sign giving them our checking account number, etc. So that they can automatically deduct an undisclosed (to us) amount. How can anyone submit such a form to them not knowing what they owe or what the monthly payment amount will be. We have written letters and called numerous times with no satisfaction. We want to pay our loan but have no information and in the meantime Sallie Mae is just adding interest to it. It's almost as if they are stonewalling people in order to hold the loan hostage and add more interest. Can anyone help resolve this?
