W-2, wages fraud

I received a w-2 from Advantech Solutions Payroll. I called them to see why I was sent a w-2. They ask for my social security number and said that the w-2 is for a company that I worked for name "Game Systems Inc." I mentioned to the lady that I worked for game systems for
only (3) days in August. She mentioned that she has records that state I worked beyond August leading into 2008 and that I cashed checks that totalled 4200.00 dollars.in which I did not recieve a check even when I had worked for those (3) days, now I am receiving a w-2 for the amount of $4200.00
