Sued Over Defaulted Emerge MasterCard, Summons Lacks Contact Info

Ok I went default on a Emerge MasterCard 6/07 when I went default cause I got cut hours at work and just have had a bad run of luck. The amount due was about 4500$. I got a notice to appear in court yesterday Emerge who I guess sold the account to Midland Funding LLC who in turn is using a lawyer to sue for 5666.63 adding more interest to it.

On the summons there is no contact info for Midland or address info. I've had contact with them before about a payment plan but they were absurdly rude and demanded full payment. I'm not sure what to do here cause I can't afford a lawyer and there is no info for Midland on the summons.
