Wrongful billing practices

This Collection Co. Claims I owe over $600 in orthopedic supplies from over 4 years ago, from a company called TMC. I requested faxes of proof and cannot get anything that is legible. They only had fague info such as physical address and not my mailing address which I have had the same mailing address for 8 years. At the time of service I used an HMO insurance company so there should not be any overages. They kept urging me to make a payment or they would report me to the credit bureau.
