Takes our money bi-monthy but doesn't pay themselves on our loan, threatening foreclosure, Columbus OH Columbus, OH

We signed up for auto-withdrawl on our mortgage 2 yrs ago with Chase. We have since, refinanced and chase bought our loan back from the new company after a few months. The bi-monthy auto payment should have continued without problems per a letter we recieved that instructed us to do nothing.
Chase has continued to deduct our bi-monthly payments from our accout, without a doubt but they are not paing themselves for the loan they hold with our name on it! Insted they call and harass and send letters threatening forclosure. None of the phone reps can get their minds around the situation so we are constantly being "transferred" to someone who can better handle the situation... But of course that ends up disconnecting us.
We are now less than 30 days away from losing the house WE ARE PAYING FOR every month!
We have also faxed every requested documentation to CHASE two times and they currently want it sent again. What can we do? We are jumping through every hoop they put up and none of it is good enough. THEY HAVE OUR MONEY in their company!!! They are ruining our credit, I'm sure.
