Never had an AT&T account with no one but its on my credit report

An Account with AT&T has been placed in my name from your company for a balance of $414.14. I am extremely confused and need your assistance in getting this problem rectified.

Please note the following:

I have never had an account wit AT&T

This appears to be a case of identity theft.

Could you please provide me with the following information:

Please provide with information regarding I need to know what year this account was opened; what address were these bills/statements being mailed to? What is the name of the company? Can you please provide me with a copy of the form that I supposedly filled out?

I just turned 20 years old and so I could and did not sign any legal contract with anyone. I need this issue resolved ASAP. Please direct / assist me in any way that you can so that I can have this issue resolved promptly.
