100% false credit reporting my credit is 100% ruined! Help!

Nco has been and is still falsely claiming and reporting that i have several unpaid MYSTERY accounts from over the last 4 years - what they apparently dont know is that i am disabled and since june of i have had continuous health insurance coverage (medicare for 80% of bill and medicaid QMB for the remaining 20% of bill any/all co-pays and/or deductibles.) that pays 100% of any and all doctor and hospital bills! My credit is 100% ruined! Please help!
