Unauthorized withdrawal from checking acccount

On July 5 my bank account exhibits a computerized withdrawal for $49.95 from "Goal Awards." I've no idea what this cost was for; who "Goal Awards" is and who approved my lender to withdraw the total amount as previously mentioned above.

I'm informing my lender to change the withdrawal and also to inform the Greater Business Bureau of Utah and document a clearly worded criticism.
