Impact Card Services - Icard Services ripoff no refund faudulent unlawful entry into bank account indescriptive in services

I thought I was applying for a credit card. When I realised that it was for a prepaid debit card, I thought I had stopped the transaction. Apperently I didn't stop it in time. I have called them numerous times, and faxed a refund request form to a fax number with a California area code. The last time I had called them, they said that according to thier records, the check had been sent in December. Now when I try to go to the web page, I get a web page not found screen. Apperently they have a new support site. I tried calling them but no response. They apperently have a new nuber. I never recieved a conformation e-mail from them. They took $159.00 out of my account without me knowing, and caused six overdraft charges (at $31 each) in my bank account.
