Failure to deduct ins. Premium as agreed

My name is Joseph Wilson I recently in June set up a payment plan for whole life insurance with this organization and gave my bank routing number, along with a voided check and a money order for the first payment of my life insurance for 79.00. The automatic bank deduction did not happen and I have contacted this organization several times explaining this situation. I was promised a form would be sent to correct this matter and I requested a refund because after not receiving the form promised I sought insurance somewhere else.
