Charged voicemail services to Qwest bill from internet completion of survey

I filled out a survey on the internet on August 14 but did not purchase any services. It was just an informational survey. They said I requested voicemail services to our phone service which I would not have done had I known because we have an answering machine. I called and cancelled their service but the gal I talked to said I completed the survey and submitted it. The only reason I submitted it was to go on for the survey, not to purchase any service. It was not stated in the survey that I was purchasing anything.
