Misused my signature

I was checking out and the sales associate asked me if I was interested in becoming a member of the VIP Club w/Suncoast Video, giving me discounts and early sale opportunities, and auto withdrawl so I wouldn't have to worry about monthly payments. I said no, paid with my bank debit card. He then asked to to sign a form verifying he talked to me about the VIP Club, which I did. It turns out I signed (under false pretense) an authorization for auto withdraw from my bank @ $9.99 a month. I have complained endlessly and nothing has changed. I am now working with my bank, but they have not been able to help. Watch-out for their sales personnel.
