It took me a while to dig through this mess, but this company apparently has relationships with online lender firms. After speaking with customer service rep, this online faxing solutions company automatically gets your checking account information if you have applied with an online loan service (not sure which one). Unless you UNCHECK the box on the form, you will automatically be enrolled in the service and billed monthly for it.

I did actually receive a confirmation number that the transaction had been cancelled and that no further charges would occur. I will wait and see what actually happens with it.
