Hacked into my back account and charged me a 29.95 fee

Today I got an email from my back saying I had insufficient funds and saw that I had a $29.95 fee taken out of my account by My Cash Guardian. I have never been to their website but somehow they got all my info and basically fraudulently charged my account a fee for something ive never even heard about. I want my money back and now im going to have to change accounts because of this! I read on a blog that this has happened to several other people without their consent or even knowing of this company and website.
