Crystal Solutions LLC. Took $39.95 out of my checking account without my authorization. I don't even know these people and don't know how they got my banking information

These people illegaly took $39.95 out of my checking account without my authorization. I don't know them and I don't know how they were able to get my routing number and checking account number. After noticing last month that there was a deduction out of my account I called them and ask them how did they get my banking information. There response was that you filled out a card or something on the internet. I told them I don't give out my banking information on line or on a card. I told them to cancel the info and return my money back to me. The person I was speaking with told me that I would receive my refund within 7 to 10 business days. Well I called on the 14th of march and spoke with a lady and begin to give me the runaround. I lost it with her because she told me that money was refunded on the 5th of March. This company has turned all there phone calls over to a customer service company and you are not able to speak with anyone from this company. I later found out that this company is also listed in the scam book. I have been in contact with my bank and they said there has been no refund to you for $39.95. Also the customer service company wanted me to fax them a copy of my bank statement. I told them you are crazy, you have already taken money out of my account without proper authorization and I'm not just going to turn over my bank statement.
