I just received the same e-mail as the others in this group, after applying to some jobs on Craig's List in Madison, WI. I do not know which posting I applied for initiated this e-mail, however I believe it was this one:

Receptionist / Office Assistant (Madison)

Property management company in search of reliable, trustworthy and customer service oriented receptionist/office assistant. Experience in the vacation rental management industry preferred.

I began filling out the form, however the second question asks what position I am applying for. Obviously I didn't know, so I googled the company (their website looked like a stock template without any updates since which was suspicious). Luckily I found these reports before going further.
