Unauthorized opening of account which I located on my credit report and was charged a fee without my consent

An unauthorized and unknown account was opened by USA Credit in my name which I recently found on my credit report as open credit. I have never received any phone calls or correspondence by mail. How could this be? I have never heard of this company. I, too was charge a $149 fee, also unauthorized.
