Card rejection

I obtained a notice within the email from your own organization in mention of the questioning me a charge card from Modells. The main reason provided was that my social protection amount could not be confirmed. I never completed to get a card, nor store at Modells. It's obvious that somebody attempted to grab my identification and obtain a card in my own name. Would you please allow me to understand where this software was completed at and so I may contact law enforcement. If this will occur again in my own title please inform me immediately. Thanks for the time and help.
