Ripoff, Took money out of my account under my maiden name, and can't tell me why Canoga Park

I have been waiting for a refund for months now for money that was taken out of my checking accout under my maiden name.

I have filled out the paperwork they sent me tha said ten (10) to fourteen (14) days to process. It has been two (2) months and they still haven't processed my refund.
I have spoken to a manager and supervisor they both can't tell me anything.

I am fed up and they should be band for selling things over the phone, Internet etc.
