Bogus $518 Judgment Reappeared on Credit During Home Refinance

This happen about a year ago, when trying to refinance our home, the judgement on my credit for $518.00 appeared. We managed to refinance only be providing information about their practices. It appeared again, no one has been able to remove this bogus claim.
