Neglegence and unprofessional

I've been seeking my clinic medical records for March 4 for 10 months and although I've been informed it'd be studied care of there's been no improvement, I had been informed by his nurse, Rita, in early Aug. That she'd visit a healthcare facility and obtain my documents. (a healthcare facility is shut but I had been provided my medical documents for March 3 inside a week). I've sent, e mailed, named, and delivered a registered notice to obtain OUR documents. On Aug. 17 I'd a doctor. Visit/pacemaker check when Dr. Asif Ali quit the area and his dad, Dr. Ali, Sr., arrived in. Besides a number of tablets that I consider he offered me examples of Tribenzor along with a software for Exforge. I'd a sign in me and so I named work and also the male nurse, title unknown, explained to not consider equally, as did the pharmacist. Dr. Asif Ali will not recognize the error and neglegence. I've emails from him declaring what he'd to but has declined to check out thru. I currently require my medical and clinic documents to become used in a brand new physician. I want a scheduled appointment soon due to medical issues and my programs working out quickly. I have had a moderate coronary attack, 2 moderate shots, arrythmia, pacemaker. Many times I actually donot feel perfectly. I want this new physician to obtain my documents ASAP and so I could possibly get treatment.

I've emails and also the physicians reactions that I will forward when required.
