Incorrectly denied / hole in buttocks

To Whom It May Concern: My name is Janice R. Butler, I am writing you in regards to the Social Security (SS) Aministration, and Medicare establishment in Arizona. I have been falsy denied coverage B.in the year 2006 I request medicare assistant. I received no response iuntill the end of the year 2008. Immediately, I phone the SS and Medicadicare and informed them I had insurance through my daughters kids Care AHCESS Program I even wrote a letter. Because I only talk of medication I did say B in the letter. I only paid AHCESS $77.00 a month for my daughter and myself and all medical was free, so why would I pay $96.40 for medicare and it only covers 80%.

Well, I the AHCESS program ended in September and I was told I'd have Medicare and the premium would be 96.40 So I paid for October November and December. Then I was told my medicare that I had to 106.40 for September. So I did. So 2009 was paid in full. But come January I got a notice I was suspended for non-payment, then reinstated after medicare found the error.

Now February came along and I receive a bil for $303. I panic, had and angina attack and ended up in the hospital. When I got out I I immediately call Medicare and spoke to a Betty and and was told pay what you could each month. Yes I sent $25,50 100 since I am on a fixed income through my California Teachers Disability retirement. (NO SS of any kind) Half is rentI And was told to go back to SS office to get B coveraged changed on computer. I went to the Social Security office and found out that I was suspended again. I informed them that I had just left the Social Security office a couple of days ago and spoKe with a Ms Deborah Kawalski and she spoke with her supervisor and they agree that I could pay $150 month. I wrote a check write there. The next month I sent 150 off and Ms Debborah said they had not received it but the third check had been cashed. Afterward, the return check came back with a letter stating wrong division, but prior to that I had wrote them another check so there would be no breackage in my coverage. I even brought that in still stapled together.

I Called Ms. Deborah again she said bring all your evidence, Now I go onto the office again and find out that they have me suspended for $1550 for non-payment of 2008. I am thinking A minor amout for 2010. I A show A Hispanic supervisor who name wasn't given that remembered I showed him I had insurance, total coverage through Kids Care AHCESS. The supervisor even opened the letter from AHCESS. He said that wasn't good enough unless AHCESS was paying my premiums. Then all he kept saying was"When medicare wants there money, they want there money, " He said that at least five times I showed him all my sufficient evidence that proved I did not even use Medicare in 2008. The Suservisor just said pay the $1500 all at one time and they will reinstate your B. Mind you IM totally disabled under the American Disability Act (ADA). And have had 5BACK SURGERIES, AN IMPLANT IN MY BUTTOCKS, I CURRENTLY HAVE ACTIVE VALLEY FEVER, ANGINA, GERD AND OVER HAVE HAD AT LEAST 30 PAIN SHOTS IN MY BACK AND BUTTOCKS.

Here is the real proble because the implant has been in so long, 6 years, its not working totally correct. And the one i have in my buttocks is 5"x5" the newer models are about 2"x2" inches. Well, what has happpened is the corner of the box has punctured a hold through to my outer skin and theres a hole allthe way down to the box from my outer skin. The dr even cancelled a surgery i was suppose to have a month ago, all because of these wrongful b coverage issues.

My fear right now is my legs are getting weaker and weaker just like they did before when I got confined to my first WHEELCHAIR FOR 2 years. The pain is not only unbearable by the tingling sensation is running down from my back to my toes.

I even went to Shea Parkway hospiital Friday September 24th and they told me the Dr. Would only do it on a 23 hr outpatient. So they doped me up with mophine and sent me home.

Finally, I have sumitted an apeal to the Social Security office aprroximately 2-3 weeks ago and no response. I am still wiling to pay my back pay for 2010 deficit, but an adjustment should be made for 2008 just like someone did for 2007. I have ran back and forth between Social Security and Medical and no one wants to help me. I need My B coverage reinstate ASAP due to the fact that I have a HOLE IN MY BUTTOCK DOWN TO THE MACHINE PRONE TO INFECTION AND ANY EXCESS MOVMENT IT BLEEDS. Please help. IM IN SEVERE PAIN!!!
