1) November 11, hurt at office, pushed to E.R.By boss. Handled for head laceration and (2) bilateral cracks to remaining and correct thumbs were xrayed and known expert.
2) December 03, Notice permitting surgery released to treating doctor.
3) December 23, Surgery middle calls to plan a 12/24/08 process and views me for pre-sugery at 2:30pm. Doctor recommends the surgery reaches greatest "extremely improbable to fix deficiencies in either flash because of substantial scar tissuing and late therapy". I delay at doctor's guidance.
4) January 14, Sedgwick problems notice moving administration of my state to Liberty Mutual/Helmsman.
5) January 20, The House Depot announces the final of its Irving service and that I am suseqently let go.
6) February 05, Follow-Up visit. Doctor performs disability examination to both thumbs and problems are accountable to Sedgwick/Homedepot that medical optimum medical enhancement have been attained which lasting disability of 6% complete body endured.
7) February 20, Ultimate evening of work at THD.
Material of complaint:
I've never noticed from the events involved with this state since my last medical visit on 02/05/09. Our criticism is the fact that I currently experience serious pain and swelling consequently of the (42) time wait in arranging my surgery for flag positioning initially decided to on 11/11/08. That I've been handled with callous neglect From The Homedepot, Sedgwick, CoCentra and Liberty Good. That I plan to follow all municipal and medical treatments available. That I'm presently searching for legal representation. That I've all documents-including unique x-rays from E.R. To verify all statements.
