Make you give up request payment by delays dose not provide claim form yet keep come up with reasons for not paying such as diagnois code missing then procedure code missing tax id now missing claim in processing after several calls you just have to give up costing more in time the the 27.00 claim I am putting.

I gusse they count on this have already made at least 5 calls I have now just gave up and will drop this hmo... By the way the tell you will never have to fill out forms the claim I put in was for brace ordered by an in plan Doctor from a surgical supply he sent me to. They don't argue that they are responseble for the charge they just make it not worth pursueing...
