I am writing to Central Coast Nutraceuticals, a company with multiple addresses ("head office" ostensibly in Arizona, but product shipped and billed from Florida). The Better Business Bureau has informed me that complaints must be filed through your Arizona office.
MY ATTORNEY HAS ADVISED ME TO DEMAND AN IMMEDIATE CREDIT CARD REFUND OF ALL MONIES CHARGED TO:

This is to serve you with notice that I have filed a complaint against your company with the Better Business Bureau regarding your fraudulent practice of shipping and billing for products not ordered, as well as hitting my credit card with a subsequent order. Along with others who are prepared to file a class action lawsuit to shut down your company, I responded to your ad for a $1.00 product sample. You charged me $44.85, subsequently hitting my credit card with an additional unauthorized charge of $39.95.

This email serves notice to Central Coast Nutraceuticals that I am demanding a full refund of the $84.80, in addition to the immediate removal of any and all of my personal and credit card information from your database. For the record, in spite of repeated calls, I was unable to get a representative on the line.

After waiting on hold for more than twenty minutes, I was asked to leave a message including my sales order number— a number that does not appear on the bill or anywhere else, for that matter. I am now pursuing this matter with my attorney: I have legal insurance that allows me to do this without any additional expense apart from my usual monthly fees. I am looking forward to taking this matter to its ultimate resolution so I will have my fraudulently extracted money refunded. I am prepared to launch a class action lawsuit and employ any other means to ensure that your company will not continue to engage in fraudulent representation to myself or any other consumer.
