Debiting my credit card without my authorization. Las Vegas

I am making this report and same time soliciting for the refund of my visa credit card which was debited to the amount $19.95 on a transaction made on 04/21 2008 which i never authorize. The transaction was about iw/living lean las vegas with a subsequent debiting of a credit card with the name "kenneth ike, " and card number "4185 8636 8501 9143." Meanwhile, I have already cancelled the above card and demanded a refund check to be sent to me via; Kenneth Ike, at 1145 University Ave, Apt 10H, Bronx, 10452, New York.
