Last November I saw the above Dr. I called before scheduling an appointment to confirm their office took my insurance. The gentleman who answered said that they do and proceeded to schedule my appointment.

I checked into the office at my appointment, filled out the paperwork, and the gentleman asked for my
insurance information. I told them that I didn't bring it with me. He said not a problem. He logged into the system, printed off my ID cards, made a copy
for their office, and gave me a copy for my records.

Never once did they say I was out of network or my insurance wasn't going to be accepted. About 2 weeks later I get a denial notice from my insurance company
stating that the Dr. Was out of network and he had just recently asked them about becoming a member of the plan I had.

Then about 2 weeks later I got a
bill from the Dr.'s office stating the balance is due. I was contacted by their office and told them I was not responsible for the bill as I asked them at least
2 times if they were in network, and they said yes.

Also, the gentleman at their office printed off my ID card, which gave them all my insurance information, and never told me I was out of network. They asked that I send them an email so the Dr. Could review
that. That was done on 2-9-07. To this date I have heard nothing from the Dr's office. I was contacted by a collection agency in September for the bill plus
interest. I have disputed the debt with them, and they have now referred this debt to a law firm for collections.
