HRS/HSA change of policy scam

We have had Optum Financial for 2 years through our employer to pay for HSA/HRA medical bills. we have been denied payments and harassed every time we try to use their portal to upload documents supporting claims. today I was told that anything prior to Aug 2021, even if ti wa on the same medical bill, was denied for being under a different plan. we were never informed that plans change over every year and there are deadlines every year for filing for the EXACT SAME SERVICE that we have had in the past. games card, same account number, same information etc. But "the plan changed" so prior year is no longer payable so "you must pay us back or we will report you to the IRS." we had plenty of funds left in last years account and those rolled over... so why can't they pay a bill from late July that is on the same bill that we received and paid in august from the same provider?
