I have called several times about not receiving an order form along with the catalog. First of all it is not an 800 number it is not the number on the new member letter, the e-mail address I finally found in the fine print is non-existant. After being put on hold for 20 minutes Michelle finally answered and told me she was faxing an order form right away, well needless to say after 3 20-30 minute phone calls no order form ever arrived.

The person that sold me the package is Jerry Boroda but of course when I ask for him they don't know anyone by that name. There should be a way that we can track the account our $200 dollars went too and put a hold on that account until we get our money back.
