I have been recieving these letters from all kinds of places. The only one that i could locate was APR. My letter went like this!!!

Your prompted acceptance of this matter is requested by returning the enclosed INDIVIDUAL VERIFICATION FORM in order that accounting may process your order of acceptance and finalize delivery of the Entrant Procedure Directives Newsletter we offer you the total amount in money and prizes!

Then they asked me to send them 31.99 with my form. For a report fee.
