Ripoff HIdden Phone Bill Charges From You Never Knew You Signed Up

My SBC bill was over charged by $12.95 plus tax, after further investigation, it was charged like this:

I immediately called them at that number, but they said they were closed, I went to their website and got the 24 hour number, they said they would cancel my membership and refund my money. I also called SBC and put on a third party block. Please beware of these companies, if anything, put in a fake phone #. I do remember signing up for some survey, but according to them I had to give my mother's maiden name, I don't remember doing that and I don't remember getting an email about it since I don't check my spam or bulk mail. I was never aware I had joined the EDN.
