Ripoff $21.95 out of my checking have all of my personal info

This company was provided my personal checking account information by another "unknown" company. They charged my checking account $21.95 which they said they would "cancel" my membership but could not reimburse me of the charge. They would not provide nor had record of HOW they obtained my personal information. The person on the phone named "Rachel" would not provide a mailing address or a location of where the company is located. Additionally she said that I was notified of my "free" 7 day trial membership by e-mail. I searched all of my e-mails and nothing from this company. No mail is sent out to notify a "customer" of this membership.

The best I can do is contact my bank and HOPE they do not continue to charge my account. Also they have all of my personal information she said, SS#, home address, birthday, etc.
