I also recieved the same letter today from USSA about the 3,700, 300.00 Failure to respond will result in permanent and irrevocable cancellation of shipment of prize documents totalling over 3,700, 300.00. Return form W-915 in my name with 20.00 dollars document fee in the prepaid envelope provided. I was almost ready to send it but I just happened to find this site and now know they are just another rip off scam artist. Worst off is look at bottom they want your personal signature, they say its required.
