Ripoff where's my rebate?

I bought a 3 piece non stick pan set from Kohl's on December 30. I sent in all of the required rebate information. The $10 rebate coupon said to allow 8 - 12 weeks.
It's been 18 weeks, still no check. No phone number. Nothing
