Patch came off

I bought a grey top having a center about the entrance. I used it once so when I cleaned it one's heart started to come off the top. Before cleaning it I adopted the instructions precisely. I'm not able to return it to Kohls since it continues to be cleaned. I'm today wondering if other people has undergone related encounters with Daisy Fuentes apparel?
