McDonalds Service: Orders, broken equipment, inadequate customer service oversight, pricing, etc

For years, I've been disgusted with McDonalds getting the orders wrong, broken ice cream machines, and employees not well trained or held accountable for their customer service. I have found this in several McDonalds in various states. Now that we are in Arizona, our local McDonalds, 91st & Union Hills Dr, Peoria, AZ, has become my last straw. Now their pricing borders on the ridiculous. They charged us 50 cents for each tomato slice, & each serving of lettuce on a burger for me...$1.00 extra for my sandwich. If McDonalds were no longer in business, the only thing I would miss is their coffee. That's all I'll be getting from them in the future... Until their coffee becomes increasingly more expensive.
