Credit Card Scam

I received a letter in the mail on December 11, saying that I was approved for a Capital Choice Consumer Credit Card for 5,000. I mailed in the required information and a check for $43.00 for priority shipping for the card on January 5. I haven't received the card yet It's been more then 3 weeks.
