Pmcclub went into my bank and took $ 99.49 without my authorization

On Aug 24th pmc club went into my checking account and took out $99.49 without my authorization. When i contacted the pmc club to inquire about the authorized transaction, the representative had all of my bank information. I was not trying to order whatever the club had to offer, i was trying to obtain a payday loan, and because of the actions of pmc club, my account went into overdrawn, i can't buy grocery nor gas to get back and forth to work. The representative told me that the only way that i can get a refund is to go to their website and print out a cancellation form, fill it out, and send the complete to them with a copy of my bank statement and that's when they will determine if i will receive a refund.
