Unauthorized charges

For that previous 5 weeks i keep receiving an unauthorized demand on my banking account for 9.95. I've approached my lender and complete the types they deliver declaring I have not authorized these costs. After doing this, I simply obtained another from my lender using the 9.95 cost again on 5/31. This keeps my banking account in uncertainty because of facility costs etc. Please stop getting me this charge when I haven't authorized this!!! Please credit my banking account using the previous costs you've obtained without my authorization!
