ULS Took 49.99 from my checking account without my authorization, liked to some payday loan application

I noticed this E-check on my account in the amount of 49.99. When I blew up the check to see the memo, it said to call 1-866-592-6448, ref#2049867, www.ULSCustService (which doesn't pull up). I called the number, spoke to a woman, no name given, the connection was very poor, but from what I heard this was linked to a payday loan application, which I did not take out.

She did cancel the account so that I would no longer be billed & gave me a ref#, I assumed that she was also crediting my account, but after reading all these reports I am going to call back & verify.

I still do not know exactly how my account information was "taken" from my application, when my application only asked if I had checking & or saving accounts & was on a "save" websight.
