While created the visit, they stated that they'll consider my extra insurance, your day of the appointment declined and that I wasn't ready to hold back for another supplier, paid $5040 for that process, can't-get anything back in the billing office.By agreement with Blue Cross allocation was $1208, my obligation is just $208.
