Charges to a nonexistent account in my name Souix Falls South Dakota

In First Premier Bank contacted me about issuing me a credit card while living in North Carolina. I never received the card, muchless used it. My credit report shows that I have a high balance of $276.00 with a max limit of $250.00. How could this be? I have never owned a credit card before and still only use my Visa check card for all my purchases, which comes directly from my checking account.

This has hurt my credit tremendously and need direction as to what I can do to get this off my credit report.
