Rip-off! Dishonest, fradulent billing! They withdrew from account without permission

I recently went to look at my checking account statement and I saw that a company had taken funds from my account without permission. So when I went to cancel or call company, I was not able to even find a phone, fax, or any useful information that would allow me to find and contact the company. For this reason, I am extremly mad!

Please help, because I have never heard of this company and I do not even know how they got a hold of my account information!
