I took my 2000 Silverado in to diagnose a hard engine code. It turned out to be a catalytic converter issue on the passenger side. I had that fixed, no problem. I called back to verify it had been repaired. I was notified at that time that the oxygen sensor on that side had the tip broken off by a piece of the bad catalytic converter. Sounded a little odd, but I ok'd the replacement of it as well. I picked up my truck that evening, drove home, worked fine.
After getting home I inspected the repairs, and low and behold, they replaced the oxygen sensor on the driver side instead of the "broke" sensor on the passenger side, which the bad catalytic converter was on. Not only did they replace the wrong one, they cross-threaded the oxygen sensor. I immediately took a picture of the obvious misthread, called the repair shop, but was closed, and left a message about the repair.
